Student Registration Upon registration you will need to bring your students certified birth certificate, a copy of your students current vaccinations and proof of residency. Proof of residency could include a phone, electric or gas bill that verifies your physical address. Kindergarten Registration will begin May 6, 2009. The student must be age five (5) by September 1, 2009. (Please see above paragraph for required documents.) If you need a certified birth certificate for your student, please use the following address to access the Arizona Department of Health Services. http://www.azdhs.gov/vitalrcd/index.htm If you have any questions regarding registration please call 567-8012 or 567-8011.
